Shayna Rogers found Trout, a severely underweight Golden Retriever, along the Trout River in Jacksonville’s Northside. He weighed about 49 pounds, and what initially looked like worms or another common illness became a months-long fight against Pythiosis.
Trout eventually reached 95 pounds, but testing took months to identify the cause of his poor condition. Biopsy results confirmed Pythiosis after a veterinarian had warned Rogers that his prognosis was limited.
Treatment included exploratory stomach surgery, multiple medications and immunotherapy. Trout also received blood transfusions, while complications extended his hospitalization by several weeks. Dr. Walker, a specialist based in Nassau County, worked with Rogers on the intensive treatment plan.
Rogers later said Trout’s Pythiosis numbers had dropped below five, a level she described as negative. That result marked substantial progress, but Trout will need close monitoring for the rest of his life.
Why freshwater exposure matters
Pythiosis is caused by Pythium insidiosum, an oomycete, or water mold-like organism, rather than a true fungus. It is associated with warm aquatic environments and especially stagnant or slow-moving freshwater.
Dogs can encounter the organism through contaminated wet environments. Research published in the Journal of the American Veterinary Medical Association identifies open-skin contact with contaminated areas as one route and says dogs may also become infected by drinking contaminated water or eating plants carrying the organism.
A review of confirmed U.S. cases found that infections in dogs and horses were concentrated in the Southeast, with Florida among the states recording the most cases. However, infections have also been identified outside traditionally recognized tropical and subtropical areas, so geography alone cannot rule out the disease.
Avoiding stagnant or questionable water is a reasonable risk-reduction step, particularly in warm conditions. Owners can also protect cuts and scrapes and check a dog’s paws and skin for injuries or irritation after walks through wet or muddy areas.
Signs that warrant veterinary attention
In dogs, Pythiosis most often affects the gastrointestinal tract or skin. The Merck Veterinary Manual lists vomiting, weight loss and loss of appetite among gastrointestinal signs. Skin disease can appear as nonhealing wounds, invasive masses or ulcerated nodules with draining areas.
Diarrhea can also occur. Rogers urged owners to seek veterinary care for vomiting, overnight diarrhea, poor eating or refusal to eat hard food, reflecting the signs she observed during Trout’s illness.
Those symptoms can have many possible causes, and owners cannot determine Pythiosis from symptoms alone. A veterinarian may use a combination of tissue examination, culture, blood testing and molecular testing to investigate suspected disease. Trout’s diagnosis was established through biopsy results after a prolonged evaluation.
Earlier assessment matters because Pythiosis can resemble other gastrointestinal or skin conditions. A recent Emerging Infectious Diseases study described cases in which skin changes were initially interpreted as inflammatory or bacterial problems, contributing to delayed diagnosis and extended treatment.
One recovery is not a treatment forecast
Trout’s combination of surgery, medications and immunotherapy should not be treated as a standard plan for every dog. The location and extent of infected tissue influence whether complete surgical removal is possible, while conventional antifungal drugs may have limited effectiveness because the organism is not a true fungus.
A peer-reviewed case series on gastrointestinal Pythiosis describes aggressive surgical removal as the most consistently effective approach, although complete removal is not always possible. Medical combinations have helped some dogs, but limited case numbers do not establish one universally successful treatment.
Immunotherapy also has limits. A review of Pythiosis immunotherapy found that it can help some affected people and animals, but not all, and its independent effect is difficult to determine when it is combined with surgery or medications.
For Trout’s family, his reported negative numbers and weight gain brought relief after months of hospitalization and uncertainty. His lifelong monitoring remains an important part of the outcome: survival does not make Pythiosis mild, predictable or safe to manage without veterinary care.
